Required Screening Questions
Do you have any of the following new or worsening symptoms or signs?
(Symptoms that are not chronic or related to other known causes or conditions.)
-Fever or chills
-Difficulty breathing or shortness of breath
-Cough
-Sore throat, trouble swallowing
-Runny nose/stuffy nose or nasal congestion
-Decrease or loss of smell or taste
-Nausea, vomiting, diarrhea, abdominal pain
-Not feeling well, extreme tiredness, sore muscles.4. Have you travelled outside of Canada in the past 14 days?
3. Have you had close contact with a confirmed or probable case of COVID-19?
Results of Screening Questions:
If you answer NO to ALL questions then you can enter the clinic.
If you answer YES to any of the above please DO NOT ENTER THE CLINIC. You should go home to self-isolate immediately and contact your health care provider or Telehealth Ontario (1 866-797-0000) to find out if you need a COVID-19 test.
COVID-19 Protocols:
At King City Imaging we have implemented increased safety measures to protect our patients and staff from COVID-19.
We regularly disinfect surfaces and the seating area.
We have installed protective barriers at our front desk.
Hand sanitizer is provided to patients and staff.
All staff, patients and accompanying persons are required to wear face masks when entering the clinic premises.
For pregnant mothers who would like to have their spouse present in the ultrasound room during COVID times, our policy is as follows:
The ultrasound tech can invite the spouse to see the baby, towards the end of the exam, after all measurements and the technical part of the exam have been completed. Please note that you will need to wear proper personal protective equipment (PPE) that will be provided to you.
